3. Determine who will implement the interventions and under what conditions

After the goals and embedded interventions have been developed, it is important to plan who will implement the interventions and under what conditions. For every embedded intervention identified, Jackie and Luke’s family need to reach consensus on who will implement it, when and where it will be implemented, and whether other children or adults will be included*. Using a Child Activity Matrix is a convenient way to visualize this plan and serves as a daily reminder to ensure embedded interventions occur.
